Acuity Water
What it does: To Find Uxie (You Must attach the Acuity Water to your starter and go to any map and find it at the bottom of the page where wild pokemons like Chimchar, Piplup, etc. appear)

Alph Glyph
What it does: Use it to Catch Shiny/Dark Unknown in Caverns of Glyph (You must Attach the Alph Glyph to your starter to catch them)

Amulet Coin
What it does: Doubling cash earned after a battle.
Note: The pokemon who is battling, when the opponent last pokemon faints, has to have the item attached.
Where to get it: Trading in Team Stat Points.
Rank: Specialist

Azure Flute: 
Have Shiny/Normal Palkia and Shiny/Normal Dialga in your roster and visit Empty Cliffs, two scientists will appear, click on them and click the link to battle. There are 10% chances of winning an Azure Flute.
After collecting six Azure Flutes visit Empty Cliffs again and click on Scientists and this time they will lead you to Arceus' tower. Beat it to get Arceus.

Black Belt: 
What it does: Powers Up Fighting Type Moves by 50%

What it does: Powers Up Dragon

What it does: Shares exp earned with the pokemon you attached the item to

Gracidea
After you beat Shaymin (for battling Shaymin see Secret Key) you get another item (The Gracidea). Attach THAT to your starter, click around the maps until you find a Shaymin, beat it, and then it's added to your box.

Griseous Orb
Search the Caverns of Alph map while Giratina is in your first slot. You will see a yellow object on the map; click it and defeat the Giratina to obtain the Griseous Orb (You need to click on the Griseous Orb when it appears, it won’t effect if you click anywhere else on the map). Attach it to Giratina and beat the Evolution Gym in order for Giratina to change into other form.

Hard Stone
What it does: Powers Up Rock

What it does: Critical hit 3/5 of the time in which you attack.

Secret Key
Battle the Team Rocket Grunt on every second Monday of the month at the Radio Tower to obtain it (small chance to obtain it). Attach the Secret Key to your starter Pokemon and start clicking around Map 1 (Dark Islands) until you come across a message that tells you to enter some sort of Secret Passage. Click that link

Sharp Beak
What it does: Powers Up Flying

Defeat the Silph Co. Only 10% chance to obtain it.

How to use: Attach it to Electabuzz and beat Evolution Gym

Smooth Pearl
What it does: Evolves Magmar into Magmortar.
How to use: Attach it to Magmar and beat Evolution Gym

Soft Sand
What it does: Powers Up Ground

Team Boost
What it does: You get 150 times more team points from beating opponents than you normally do

Twilight Orb
What it does: Unlocks Unown Tower Phase #1.
Where to get it: Attach 6 rare metals to your roster, have Fire type  Pokemon as your starter and beat evolution gym to get Twilight Orb.
How to unlock phase: Just have your starting pokemon with the item attached and go to the Unown Tower.

Twilight Rod
What it does: Unlocks Unown Tower Phase #2.
Where to get it: Attach 6 rare metals to your roster, have Water type  Pokemon as your starter and beat evolution gym to get Twilight Rod
How to unlock phase: Just have your starting pokemon with the item attached and go to the Unown Tower.

Twilight Rod + Twilight Orb
What it does: Unlocks Unown Tower Phase #3.
How to unlock phase: Just have your starting first two pokemon with the items attached and go to the Unown Tower

Twisted Spoon
What it does: Powers Up Psychic + Evolves some Pokemons
